Type 2 diabetes mellitus DM is an important contributor to most
manifestations of cardiovascular disease including coronary heart disease,
cerebrovascular accident and heart failure Clinical trials have
demonstrated that awareness and control of DM can reduce or even eliminate
these multiple serious morbidities This report summarizes the prevalence
of DM in a large sample of adult African Americans who participated in the
first clinical examination, testing and interviews of the Jackson Heart
Study Blood was drawn after an overnight fast and interviews and
anthropometry were performed at the clinic American Diabetes Association
ADA 1997 criteria fasting plasma glucose level ?126mg/dl were used to
define the prevalence of DM which varied from approximately 8 44 of 563
in younger women age 35-44 to a high of 291 174 of 599 in older women
age 65-74 Awareness of DM
was uniformly high; approximately 88 835 of
947 of participants who had elevated plasma glucose levels ?126mg/dl had
been told by a physician that they had DM: highest for women age 65 and
older over 94 [209 of 222] and lowest for men younger than 55
approximately 80 [76 of 94] Overall, about 60 445 of 747 of those
treated for diabetes manifested hemoglobin A1c HbA1c levels in excess of
the level recommended by the ADA ? 7 indicative of less than optimal DM
control: mean 78 median 74 for those treated for DM vs 83 median
77 for those untreated This representative sample of African Americans
from a predominately urban southern US population suggests a high
prevalence of DM with sub-optimal control in a significant fraction of the
population
