The Kokomo coalition has implemented a comprehensive care management
program for diabetes The program includes physician and patient
education, screenings to identify undiagnosed, sponsorship of a support

The Clinical Practice Committee has encouraged the use of best practice
guidelines for care for diabetes for several years The committee
distributed guidelines published by the state health department and check
sheets for use in patient charts to physicians in the community A
physician CME on diabetes prevalence and management was conducted in
partnership with the County Medical Society
The diabetes guidelines are also contained on patient pocket cards that
have been distributed in physician offices, pharmacies, work sites, and
health fairs The pocket cards help both patients and physicians keep
track of the screenings and tests that are needed on a regular basis

Since 2001, the frequency of Hba1c testing in the community has increased
from 83 to 84, the average value of Hba1c tests has declined from 727
to 670, the percentage of tests under 700 has increased from 551 to
70, and the percentage of tests over 95 has declined from 127 to 4
All measures indicate an improvement in diabetes management and control
The coalition screened over 5,300 people last year with 12 testing
positive for diabetes For DaimlerChrysler, inpatient admits and
payments per 1000 in Kokomo have been well below corporate averages,
resulting in savings of approximately 232,000
since 2001
