RE: Diabetic Sense Prescription Program
Catalyst Rx is the Pharmacy Benefit Manager for the Bradley University
Health Plan Effective immediately, Bradley University offers Diabetic
Sense, A Catalyst Rx Program to all health plan participants with
diabetes Catalyst Rx partners with Liberty Medical Supply to manage this
program
In the near future, Catalyst Rx will mail an enrollment brochure to
individuals using diabetic supplies The brochure will explain the benefits
to becoming a program member Participation in this program is free and
voluntary as part of the pharmacy benefit coverage The co-pay for
diabetic supplies for participants in the Diabetic Sense Program are 20
for 90-day supply via mail order versus 50 co-pay for mail order if not
in the program

could meet the needs of informed diabetics The program will assists in
controlling diabetes through proper blood glucose self-monitoring, support
and education in these ways:

Refill reminders
Free educational materials
Quarterly telephonic testing reminders eye and foot exams, A1c
testing, etc

Sense program Becoming educated about the valuable resources available
through this program is one of the first steps in learning to manage
diabetes for a healthy lifestyle To participate in this program, please
complete and return the enrollment application that will be mailed from
Catalyst All information is kept confidential
There will be an informational meeting about the Diabetic Sense program at
9:30 am and at 12:30 pm during the Benefit Fair on Friday, August 24,
