Leadership Team of the PA Diabetes Action Partnership
The first meeting I attended as an elected member of the Executive
Leadership Team ELT of the PA Diabetes Action Partnership PDAP was held
on January 28, 2008 Unfortunately there was a major snow and ice event so
only about ten of us made it to the meeting Our meeting was mainly an
orientation to the work of PDAP and a chance to hear the different
committee reports Everyone who has participated to date should be very
proud of their work as the Governor has since released his Chronic Care
Commission report and a large section of it was taken from the work of

As someone who spends most of her time in one-to-one sessions with
patients, getting to spend time with Medical Directors of the major
insurance companies and with other professionals interested in changing our
medical model to one of prevention was an exhilarating change of pace I
found all of the people involved in PDAP
to be approachable and interested
in change
One of the major points I was able to make, along with the help of the
other educators, was that when working with someone to change long-
standing habits, three or four visits over the course of a year is a drop
in the bucket and that we need to look at a provider system where a person
could get coaching weekly or bi-weekly to make lifestyle changes
Our next meeting will be held on Thursday, April 17, 2008 During the
meeting, we will first receive an update on the implementation the
Pennsylvania Diabetes Action Plan Then, we will hear about the plans of
the Governors Chronic Care Commission and how its work will impact the
implementation of the Diabetes Action Plan We will hold a face to face
meeting of the four PDAP Work Groups: Surveillance, Standards of Care,
Health Policy, and Evaluation to discuss the action steps under each of the
four corresponding components of the Action Plan
