Diabetes: What State Health Departments Are Doing
State Health Departments conduct a wide variety of efforts to prevent and control diabetes Here are just a few examples:
Determine the scope and magnitude of the states diabetes burden Establish and maintain a diabetes surveillance system to measure death, disability, healthcare access, and healthcare practices that are essential to preventing complications of diabetes
Develop and distribute culturally relevant diabetes educational materials, media campaigns, and other community outreach efforts Target most vulnerable populations with educational messages
Use networks and coalitions within communities churches, businesses, civic and volunteer groups to educate people with diabetes about the importance of monitoring and seeking treatment for foot or other extremity concerns, regular eye exams, physical activity, and healthy eating
Provide technical assistance to
health provider groups community health centers, HMOs, and family practice groups to assess their current diabetes self management programs and ensure that their diabetes care practices are consistent with the American Diabetes Associations standards of care
