from her home reservation 2 years ago where she worked for the Tribal
Health Department as a secretary for many years She now lives with her
daughter and her 3 young grandchildren
CC presents to your office with the need of prescriptions for her diabetes
medication She is accompanied by her adult daughter Her daughter tells
the nurse that CC has been experiencing headaches and tightness in her
jaw for the past 2 weeks
CC has a past medical history of type 2 diabetes since age 46, high blood
pressure, elevated cholesterol She has not smoked for 10 years, prior was
ppd/20 years Past family history positive for type 2 2 brothers, 1
sister, breast cancer maternal aunt, and stroke father
CC is fluent in her native language; English is her second language She
is a widow
CC mobility is challenged because of a chronic knee condition, the result
of a MVA 25 years ago - she uses a cane She does not drive
