Pathways to Excellence Program-DIABETES
Practices that score 75 points or more using the PTE criteria outlined
below will be eligible for Blue Ribbon designation The target will be
lowered by the Steering Committee if less than 25 of Maine practices are
able to achieve it The target will not be raised for 2006
Criteria are the same as in 2005 PTE Program, based on Bridges to
Excellence/NCQA Diabetes Physician Recognition Program goals and targets

NCQA Diabetes Physician Recognition Program

Alternatively, practices can receive a PTE Blue Ribbon for diabetes care
on the MHMC website if they achieve certification in the NCQAs Diabetes
Physician

Since this designation is for each physician, in order to receive a Blue
Ribbon a number of physicians in a practice who together provide care for
over 80 of the practice patients with this condition must receive the
NCQA Recognition Award
In order to fulfill this requirement practices must have documentation of
having submitted their data to NCQA by November 1, 2006 Due date is
extended because no validation is needed for this approach
Documentation of attaining the NCQA Diabetes Recognition award must be
submitted to the MHIC by December 1, 2006 for posting in PTE public
reporting Per NCQA posted information, the turnaround time for their
validation of a COMPLETE submission is usually one month
