Describe Type 2 diabetes and the importance of optimal blood glucose control to protect future health Explain the basic components of diabetes treatment including monitoring, meal planning, physical activity, medications and weight loss Identify a personal goal and list strategies to achieve the goal Individuals with Type 2 diabetes with a family or support person are welcome to attend Participants can be referred by their healthcare provider or self-referred A minimum of 10 participants is required to hold the program, and a maximum may depend on the location
