The Honorable [name of your Congressman]
United States House of Representatives

Dear Representative [last name of your Congressman]:
My name is [your name], and I live in [your town] I am writing to
request your support for increased funding in fiscal year 2007 for the
National Institutes of Health NIH, and in particular, for the National
Institute of Diabetes, and Digestive, and Kidney Diseases NIDDK
The NIDDK funds research on two diseases that affect the functioning
of the kidney: Nephrotic Syndrome NS and Focal Segmental
Glomerulosclerosis FSGS Nephrotic Syndrome occurs when the filters of
the kidney malfunction, causing protein to be lost through the urine Once
protein is lost, swelling occurs in the body, especially around the eyes
and the limbs, and the patients cholesterol soars Long-term Nephrotic
Syndrome can cause FSGS, which in turn leads to kidney scarring Patients
with these diseases can sometimes go into remission, but if the kidneys
fail, then they must go on dialysis or receive a kidney transplant
Physicians and researchers have not yet been able to determine why patients
develop these conditions
I write
to you because [explain how FSGS and NS has affected you and
your family]
