Diabetes
In the case of Diabetes, the information in this packet is crucial
This health information will be held confidential, available only to
personnel who are directly involved with your child
Also, attached is the NEW state School Medication Prescriber/Parent
Authorization form dated 9/11/07 You are required to have a form for
EACH medication DO NOT put two or more medications on the same form
You may copy the form as many times as needed You may also obtain the
form on the Huntsville City School Website
We have changed the forms this year, to make them more user friendly
IHP instructions:
-Parent / Guardian will complete Section I, sign and date
-Physician will complete Section II, sign and date
It would save time and help your physician if you would complete Section I
before your appointment Your physician must complete Section II and he /
she MUST sign and date
Options: Forms may also be completed by a PA or NP
NO medication will be given without the appropriate forms being completed
Bring medications directly to THE
SCHOOL NURSE or DESIGNATED medication
assistant
If you have received this packet in error, or if your students Diabetes
does not require emergency planning, please make a note on this cover
letter and return it to the teacher with your student
We appreciate your time and help in this matter
