Memorandum of Understanding for a Partnership between the Royal Danish
Ministry of Foreign Affairs and the World Diabetes Foundation

The World Summit for Sustainable Development to be held in Johannesburg on
26 August - 4 September 2002 will emphasise innovative Partnerships at all
relevant levels as a crucial means for achieving sustainable development

The Royal Ministry of Foreign Affairs through Danish International
Development Assistance Danida and the World Diabetes Foundation WDF
have a common desire and purpose to improve the health standards of poor
people in developing countries

As a first step towards a Partnership, Danida and the World Diabetes
Foundation have agreed to sign a memorandum of understanding in order to
utilize the respective competencies and strengths for purposes, that have
high priority in Denmarks development policy and which contribute to the
realisation of WDFs objectives

Approximately 150 million people around the world have diabetes According
to the World Health Organization WHO, in a decade that number may grow to
more than 250 million While diabetes will continue to be a major
problem
for the developed countries, it is estimated that approximately 70 of all
new cases will appear in the developing countries In the developing
countries, less than half of the people with diabetes are diagnosed
Without a timely diagnosis and adequate treatment, diabetes will most
likely rise exponentially

Type 2 diabetes, one of the major forms of diabetes, increasingly affects
people in low and middle income countries, including the poorest urban
areas, where there is lack of knowledge about or means to afford proper
nutrition and a healthy lifestyle Studies show a possible correlation
between chronic malnutrition during childhood and later development of Type
2 diabetes Lack of awareness and knowledge may result in late diagnosis
when complications are already established These complications are often
expensive to treat, and they may lead to reduced productivity In a setting
of limited resources this can bring individuals and families into debt and
poverty

To help meet the needs of disadvantaged people prone to diabetes, Novo
Nordisk has established the World Diabetes Foundation - a fully
independent
charitable trust This foundation aims to improve diabetes prevention and
care for disadvantaged people through assistance to education, capacity
building, distribution and procurement of essential drugs The WDF
currently holds DKK 500 million to be committed for projects during the
coming ten years

Under its overall objective of poverty reduction Denmarks development
policy as formulated in Partnership 2000 attaches a high priority to
strengthening the health sector in developing countries Danida supports
health sector programmes at present in six developing countries as well as
a number of health related activities in other countries The focus is
mainly on primary health care and development of health care systems
capacity

Now, therefore, the Ministry of Foreign Affairs and WDF have agreed to sign
the following Memorandum of Understanding:

Scope and objectives
Consistent with the decisions of the board of the WDF and Denmarks
development policy, this sets forth the terms and conditions that will
guide the Partnership

In support of national efforts undertaken by developing countries to
reduce
poverty through the improvement of health standards the two Parties agree
to utilize the competencies and strengths of the WDF and the Danida
respectively in pursuit of a common goal to improve health conditions in
developing countries through activities that prevent diabetes and its long
term complications This agreement will be implemented through a broad
range of interventions in order to strengthen the health systems with a
focus on prevention in the primary health sector and increased knowledge of
diabetes The two Parties aim initially to explore the possibility of
establishing two projects during the coming year in the following
countries: India, Uganda, Ghana, Kenya, Tanzania or Bhutan

The Parties agree that:
The overall goal is to contribute to prevention and limitation of the
spread of diabetes through assistance to capacity building of health
sectors Assistance will be based on the principles of national
ownership of the collaborating country; and on early diagnosis and
prevention of late complications, which may contribute to reduced
productivity and poverty;

Activities must take place within established national frameworks and
relevant sector plans and must respect the priorities laid down
therein;
Efforts will build upon and be coordinated with relevant aid
activities, including, sector programmes/projects supported by the
Danida;
Project activities must take into account the capacity in the health
sector of the collaborating country as well as risks that project
activities may distort other efforts in the health sector

The supported activities may include eg:
Building capacity within the primary health service;
Documentation, analyses and research on diabetes related problems in
an aid context;
Information about nutrition and support for preparation of strategies
for improved nutrition and healthy lifestyle, including scaling-up of
pilot projects;
Education;
Distribution and procurement of essential drugs as part of supported
projects;
Monitoring

Possible collaboration partners include:
National authorities in developing countries;
National and international NGOs;
National and
international research institutes;
International organisations UNICEF, IFPRI, WHO, etc;
Business

The WDF is responsible for identification and selection of projects,
including technical appraisal in accordance with the criteria for project
support as laid down by WDF WDF will negotiate directly with its possible
collaborating partners the necessary administrative and financial control
mechanisms for project implementation

The WDF will make the required financial resources available for
identification, preparation, implementation and monitoring of projects
approved by its Board

Danida may provide assistance as follows:
Build up the capacity of national health care systems, which will
enable these to implement national health programmes in other fields,
including combating diabetes Primary health care systems may provide
information on correct and adequate nutrition, assist in early
detection of diabetes and advise on appropriate diet for those
affected
Utilise its knowledge of local conditions and contacts to identify
possible collaborating countries and local
partners governmental
bodies, NGOs, international organisations, research institutes and
other relevant organisations;
Identify consultants, who may assist in identification, preparation
and implementation of projects and activities for possible funding by
WDF;
Share its experience from programme co-operation countries in order to
provide information on actual development challenges and problems
generally and during identification, preparation, implementation and
completion of projects funded by WDF

Participation in joint identification, preparation or monitoring of
projects will be considered

The Parties will keep each other informed, as appropriate, of relevant
activities with a view to maximizing their possible synergy

Entry into force and termination of the Memorandum of Understanding

This Memorandum of Understanding shall enter into force on the date of its
signing The Parties wishes to continue to develop the Partnership and may
at any time agree on further modalities to strengthen the Partnership
Either party may terminate the Memorandum of Understanding within 30 days
by
written notice to the other Party

Any obligation arising from this Memorandum of Understanding will be
subject to confirmation by the Board of WDF or for Danida by the Danish
financial authorities

In witness whereof, the undersigned have signed the present Memorandum of
Understanding in the English language in two copies
